Irregular periods and ovulation time

A period usually follows hormonal changes after ovulation, but the interval before ovulation can vary widely. Some irregular cycles involve Anovulation, meaning an egg is not released. This can make fertile timing difficult to identify and may cause long gaps followed by unexpected bleeding. Polycystic Ovary Syndrome may present with irregular ovulation and periods, but irregular timing alone does not prove PCOS. Nexplanon and other hormonal contraception can also alter bleeding and suppress ovulation.

Record bleeding dates, cervical mucus changes, symptoms, contraceptive use, and any ovulation tests, while remembering that a calendar prediction may be misleading when cycles vary. If pregnancy is desired, discuss tailored fertility advice rather than relying only on an app. If pregnancy is not desired, use contraception consistently because unpredictable ovulation can still occur. A clinician may assess thyroid or reproductive hormones and discuss options such as the Combined Pill when pregnancy is not currently planned; Metformin for PCOS may be relevant only after assessment.

Arrange a review if periods repeatedly disappear, bleeding is heavy or prolonged, pain is severe, or you have symptoms such as new facial hair, persistent acne, nipple discharge, headaches, or vision changes. Seek urgent help for severe one-sided pelvic pain, fainting, or possible pregnancy with pain or bleeding. Perimenopause and Premature Ovarian Insufficiency require clinical context and are not diagnosed from uncertain ovulation timing alone.
