Irregular periods while breastfeeding

Hormones involved in milk production can suppress ovulation, particularly when feeds are frequent. As feeds become less frequent, night feeds change, or solids and formula are introduced, bleeding may return and remain unpredictable for some time. A first period may differ from earlier cycles. Breastfeeding-related changes do not exclude pregnancy, and irregular bleeding may also have another cause such as Anovulation or Polycystic Ovary Syndrome.

Note feeds, spotting, bleeding, pain and any change in milk supply. Take a pregnancy test if pregnancy is possible, even without a regular period. If you do not want pregnancy, discuss contraception that fits breastfeeding and your health; the Combined Pill may not be suitable at every stage or for every person. Seek advice before using medicines for bleeding or pain.

Arrange prompt review for heavy or prolonged bleeding, fever, foul-smelling discharge, worsening pelvic pain or bleeding after sex. Seek urgent help for severe pain, fainting, or bleeding with a positive or possible pregnancy. A sudden change should not automatically be attributed to breastfeeding.

Ask whether your bleeding fits breastfeeding-related hormone changes, when to test for pregnancy, which contraception is compatible with feeding, and whether other causes need assessment.
