What spotting may indicate

Spotting is lighter bleeding than a usual period and may occur around an ovulation change or after starting, stopping, or missing hormonal contraception. Anovulation can make the uterine lining shed unpredictably. Polycystic ovary syndrome and perimenopause are other possible contributors; premature ovarian insufficiency may also alter bleeding. Pregnancy, infection, irritation after sex, polyps, or other gynaecological causes need consideration. Spotting alone does not establish its cause.

How to prepare for assessment

Note the colour, amount, number of days, relation to sex or contraception, pelvic pain, discharge, fever, and whether pregnancy is possible. Take a pregnancy test when relevant. Arrange a routine appointment if spotting recurs, follows sex, continues, or accompanies cycle changes. The clinician may ask about medicines and contraception and may offer an examination, pregnancy test, infection testing, blood tests, or ultrasound. Do not douche or use unprescribed vaginal treatments while waiting for advice.

When to seek care

Get urgent help for heavy bleeding, faintness, severe pelvic pain, fever, or one-sided pain when pregnancy is possible. Prompt assessment is also appropriate for bleeding after menopause or repeated bleeding after sex.
