Short answer

Irregular periods after childbirth are often linked to changing hormones, breastfeeding, or contraception, but pregnancy and other causes still need consideration.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why cycles change after birth

The return of periods varies after delivery. Breastfeeding can delay ovulation and make bleeding unpredictable, while reducing feeds or stopping breastfeeding may alter the pattern again. The first bleeds may differ in flow or duration from previous cycles. Contraception, including hormonal methods, can also cause spotting or lighter bleeding. A new pregnancy remains possible before a regular period returns. Persistent irregularity may sometimes reflect anovulation, thyroid problems, PCOS, or another condition rather than postpartum adjustment alone.

What you can do now

Record bleeding, feeding changes, contraception, pain, temperature, and unusual discharge. Take a pregnancy test if there has been sex without reliable contraception, even if you are breastfeeding. Arrange a postpartum review if the pattern is worrying or interferes with daily life; the clinician can discuss contraception, anaemia symptoms, and whether tests are needed. Read about [Irregular Periods](/symptoms/irregular-periods/) and [Combined Pill](/treatments/contraception-and-family-planning/combined-pill/).

Warning signs

Seek urgent care for sudden heavy bleeding, faintness, severe abdominal pain, fever, or foul-smelling discharge. Prompt advice is also needed for bleeding with a positive pregnancy test, one-sided pain, or shoulder-tip pain. Contact a clinician for bleeding after sex, ongoing fatigue, palpitations, or a cycle pattern that does not settle as feeding and contraception stabilise.

Useful questions

Could breastfeeding or my contraceptive explain this pattern? When should I test for pregnancy? Do I need an examination or blood tests? Which contraception is suitable while breastfeeding? How can I recognise bleeding that needs urgent care?
