After a meal, digestion changes blood flow and activates nerve signals that can make the heartbeat feel more noticeable. Large meals, dehydration, alcohol, caffeine and very sugary foods may also trigger a racing, fluttering or uneven sensation in some people. The feeling may be palpitations rather than a dangerous rhythm, but the sensation alone cannot identify the cause.

Atrial fibrillation is one rhythm problem that can cause an irregular pulse and may be noticed around meals, although a post-meal sensation does not by itself establish that diagnosis. Other explanations include extra beats, an overactive thyroid, anaemia, fever, medication effects or anxiety. A clinician may check your pulse, medicines and health history and arrange an Electrocardiogram to capture the rhythm.

What to do next

Sit quietly, breathe normally and avoid more caffeine, alcohol or nicotine while the sensation settles. Record when it starts, what you ate and drank, how long it lasts, whether the pulse feels fast or uneven, and any associated symptoms. Do not stop prescribed heart medicine or begin a blood thinner without medical advice.

Arrange a medical review if episodes recur, last longer than a brief spell, occur at rest, or are becoming more frequent. Bring your symptom record and a list of medicines, supplements and recent illnesses. If an abnormal rhythm is confirmed, treatment depends on its type and your risk factors; options may include rhythm or rate management and, for some people, Anticoagulation Management.

Seek emergency help for an irregular heartbeat with chest pain or pressure, severe shortness of breath, fainting, new confusion, one-sided weakness, or difficulty speaking. These symptoms can signal a serious heart or circulation problem. Dizziness or unusual weakness during an episode also warrants prompt medical advice, especially if it is new or worsening.

Do not drive yourself if you feel faint, confused or severely unwell. A rhythm problem can sometimes reduce effective blood flow, and atrial fibrillation is associated with complications such as Stroke or Transient Ischaemic Attack. Heart Failure symptoms, including marked breathlessness or sudden swelling, also require prompt assessment.
