What should you know about intense stiff neck pain?

Short answer
Intense stiff neck pain may come from muscle strain, joint irritation, or nerve involvement and should be judged alongside warning signs.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the pattern can mean
A stiff neck after an awkward sleep position, prolonged screen use, or a sudden movement often involves tight muscles and restricted movement. Neck pain can also accompany cervical spondylosis, a disc problem, or irritation of a nerve; these may cause pain spreading into the shoulder or arm, tingling, numbness, or weakness. A collision or fall raises concern for whiplash or another injury. The timing, movement that worsens it, and any arm or neurological symptoms help a clinician narrow the possibilities.
What to do now
Keep the neck gently moving within a comfortable range rather than forcing a stretch or holding it rigidly for long periods. Adjust screens so your head is not repeatedly bent forward, change position often, and use a pillow that keeps your neck comfortable. Heat may ease muscle tightness for some people; a wrapped cold pack may help after a recent strain. Use medicines only as directed and check with a pharmacist or clinician if you have health conditions or take other medicines.
When to seek care
Seek urgent care after a significant fall, collision, or blow, especially if you cannot move your neck normally. Get emergency help for neck pain with new weakness, loss of coordination, severe headache, fainting, confusion, trouble speaking, or loss of bladder or bowel control. Fever with marked neck stiffness or feeling severely unwell also needs urgent medical assessment. Arrange prompt review if pain travels down an arm or symptoms are worsening.
Questions for your doctor
Ask whether your symptoms fit a muscle strain, nerve irritation, or another neck condition; whether an examination or imaging is appropriate; and which movements are safe. Ask how to set up your workstation, when physiotherapy may help, and which changes should prompt urgent reassessment.

Questions people ask
Can screen use cause a stiff neck?
Long periods with the head bent forward can overload neck muscles and joints. Changing position and improving screen height may reduce strain, but persistent or intense symptoms need assessment.
When is stiff neck pain an emergency?
After major trauma, or when it occurs with weakness, coordination problems, severe headache, confusion, or fever and marked stiffness, seek urgent care.
