In ectopic pregnancy, why can a urine test be negative?

Urine tests look for human chorionic gonadotropin, a hormone made during pregnancy. Early pregnancy may produce too little hormone for a home test to detect. A test can also be less reliable after drinking a lot of fluid. An ectopic pregnancy develops outside the main cavity of the uterus, most often in a fallopian tube, and hormone levels may rise differently from an uncomplicated pregnancy.

Pelvic pain, vaginal bleeding, shoulder-tip pain, dizziness, or fainting need medical assessment even if the urine test is negative. Pelvic pain has many possible causes, including pelvic inflammatory disease, but symptoms cannot identify the cause on their own.

Contact a maternity or emergency service promptly if you have pain or bleeding and pregnancy is possible. A clinician may repeat pregnancy testing using blood, examine you, and arrange an ultrasound. A transvaginal ultrasound can help locate a pregnancy and assess the pelvis, although very early findings may be unclear. Do not drive yourself if you feel faint. Keep a note of when symptoms began, the test result, bleeding, and any pain on one side.

Go to emergency care now for severe or worsening abdominal or pelvic pain, heavy bleeding, fainting, marked weakness, dizziness, or pain at the shoulder tip. These symptoms can signal internal bleeding and require immediate assessment. Seek same-day advice for persistent one-sided pain, new bleeding, or a positive pregnancy test with discomfort.

Ask whether you need a blood pregnancy test, repeat testing, or a transvaginal ultrasound; what symptoms should trigger emergency care; and how follow-up will confirm that the pregnancy is in the uterus or has resolved.
