If you have an ectopic pregnancy, will a test be positive?

Home and clinic pregnancy tests detect human chorionic gonadotropin, a hormone made during pregnancy. An ectopic pregnancy can therefore make the test positive. A negative result can happen very early, so timing and repeat testing may matter.

A urine test cannot distinguish an ectopic pregnancy from a pregnancy inside the uterus. Clinicians may use repeat blood tests to assess hormone changes and a transvaginal ultrasound to look for the pregnancy and check for bleeding. Pelvic pain or vaginal bleeding should be reported even when the test result seems reassuring.

Contact a maternity or early-pregnancy service for advice, especially if you have pain or bleeding. Keep every appointment for blood tests and scans, because the location may not be visible immediately. Do not use a home test result alone to decide that the pregnancy is developing normally.

Your clinician will explain whether observation, medicine or surgery is appropriate if an ectopic pregnancy is diagnosed. A salpingectomy may be used to remove an affected fallopian tube. Ask how treatment will be followed until the pregnancy hormone has cleared.

Seek emergency care for sudden severe pelvic or abdominal pain, shoulder-tip pain, fainting, collapse, heavy bleeding, severe dizziness or weakness. These symptoms can indicate internal bleeding and need immediate assessment, regardless of whether a home test is positive or negative.

Ask whether the pregnancy has been seen in the uterus, when blood tests should be repeated, and when an ultrasound is useful. Also ask which symptoms require emergency care and what treatment would be offered if an ectopic pregnancy is confirmed.
