Symptoms to recognise

Headache may be daily, worse on waking or aggravated by coughing, straining or bending. Pulsatile tinnitus sounds like a heartbeat in the ear. Brief episodes of greyed or blurred vision can occur when posture changes, and some people develop blind spots or double vision. An eye examination may show swelling of the optic discs even before a person notices major sight loss. These symptoms have many possible causes, so they do not establish idiopathic intracranial hypertension without examination and testing.

How to respond

Arrange a clinical review for persistent new headaches, heartbeat-like ear noise or repeated visual dimming. Keep a symptom diary with timing, triggers, duration and which eye is affected. Bring your medicine list and mention recent weight change, sleep-related breathing symptoms and any history of hypertension. An eye examination and neurological assessment can help determine whether the optic nerves are under pressure and whether imaging or fluid testing is needed.

Urgent warning signs

Seek emergency care for sudden or rapidly worsening vision loss, a sudden severe headache, new weakness, facial drooping, trouble speaking, confusion, seizure or repeated vomiting. These symptoms can signal a neurological emergency, including stroke or transient ischaemic attack, or another cause requiring rapid treatment. Do not drive yourself if your sight or alertness is impaired.
