What should you know about hydrotherapy and massage?

How the approaches differ

In a hydrotherapy pool, buoyancy can reduce the load through a painful joint while warm water may make gentle movement easier. Massage works on soft tissues with techniques such as stroking, kneading or sustained pressure. It may temporarily ease muscle tightness, but it does not repair every cause of pain. These approaches can be used alongside an assessment and an active rehabilitation plan for joint pain or stiffness. They may be considered for people living with fibromyalgia, frozen shoulder or ankylosing spondylitis, depending on symptoms and treatment goals.

Planning a session

Tell the physiotherapist about recent injuries, skin problems, dizziness, reduced sensation, heart or breathing conditions, and medicines that affect bleeding or alertness. Wear secure swimwear for pool work and arrive hydrated. During either treatment, describe sharp pain, unusual tingling, breathlessness or feeling faint. Progress should be guided by how you move afterwards, not by tolerating strong pressure. Ask whether massage should come before or after exercise therapy, and whether a hydrotherapy pool is suitable for your balance and confidence in water.

When to get medical help

Stop and seek prompt medical advice for new weakness, loss of bladder or bowel control, severe swelling, a hot red joint with fever, chest pain, or trouble breathing. Arrange an assessment when pain keeps worsening, movement is becoming more limited, or symptoms follow a significant fall or injury. A treatment session should not delay evaluation of a new unexplained symptom.
