Short answer

To reduce period cramps at night, use gentle heat, a comfortable sleep position, and suitable pain relief started early in the evening.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why cramps can disturb sleep

Womb contractions can cause aching or wave-like pain in the lower abdomen, back, or thighs. Lying still may make the discomfort feel more noticeable, while poor sleep can lower your tolerance for pain. Night-time cramps that are unusually severe, last beyond the period, or steadily worsen may be linked with endometriosis, adenomyosis, uterine fibroids, an ovarian endometrioma, or pelvic inflammatory disease. Cramps alone cannot tell you which condition, if any, is present.

Build a bedtime plan

Place a warm, not hot, heat pack over the lower abdomen or back for a short period before sleep, protecting your skin. Try side-lying with a pillow between your knees, or lie on your back with support beneath your knees. Gentle stretching, a slow walk, a warm shower, and calm breathing can relax tense muscles. If safe for you, take an appropriate anti-inflammatory pain medicine early in the evening and follow the label or prescription. Ask about the combined pill or hormonal coil if cramps recur each cycle and affect sleep.

When to seek care

Seek urgent care for sudden intense pain, fainting, fever, vomiting, heavy bleeding with weakness, or pain when pregnancy is possible. Arrange a review if night pain repeatedly wakes you, is not controlled by a suitable plan, occurs outside periods, or is accompanied by unusual discharge, pain during sex, or bleeding between periods.

Questions for your doctor

Ask whether the timing or severity suggests a pelvic condition, which medicine is safe with your health history, and whether hormonal treatment could help. Ask how to use heat safely and what change should trigger follow-up.
