Why exercise may help

Sciatica is pain or altered sensation travelling along the sciatic nerve pathway, often from irritation of a nerve root in the lower back. Exercise may help you maintain spinal and hip movement, improve trunk and leg strength, and reduce fear of normal activity. There is no single routine that suits every pattern of leg pain.

A physiotherapist may assess strength, reflexes, sensation, walking and which positions ease or aggravate symptoms. Improvement is better judged by walking, sitting, sleep and daily tasks than by pain alone.

Staying active safely

Keep changing position and continue manageable daily activity rather than staying in bed for long periods. Your plan might include gentle mobility, repeated movements, trunk or hip strengthening and graded walking. Use the response over the following hours to guide the next session, and avoid forcing a movement that clearly spreads or intensifies leg symptoms.

Technique and pacing matter. A clinician can show you how to bend, lift and return to work or sport in stages. If the plan repeatedly increases symptoms, contact the clinician who prescribed it.

When to seek care

Seek urgent medical assessment for new bladder or bowel difficulty, numbness around the saddle area, or worsening weakness in both legs. Prompt review is also needed for rapidly increasing leg weakness, fever, unexplained illness, major trauma or severe pain that does not ease in any position.
