How common is cough in croup?

Cough is a very common presentation of croup, often sounding harsh or barking because swelling affects the upper airway. The cough may be more noticeable at night or when a child is upset. Croup can also cause a hoarse voice and a noisy, high-pitched sound when breathing in. A cough on its own does not establish the cause, so the child's breathing and general condition matter as much as the sound.

In croup, inflammation and swelling around the voice box and windpipe narrow the space for air to move. This can produce a distinctive barking cough, hoarseness, and noisy breathing called stridor. Symptoms may change over a short period, especially if crying increases the work of breathing. Many children remain alert and drink fluids, but a child who is tiring, struggling, or becoming unusually quiet needs prompt assessment.

Other illnesses can also cause cough, including infections affecting different parts of the breathing system. The cough's sound is useful information, but it should be considered alongside fever, breathing effort, hydration, alertness, and how quickly symptoms are changing.

Keep the child calm and upright, because crying can make noisy breathing worse. Offer frequent small drinks if swallowing is comfortable, and stay nearby to observe breathing. Follow any treatment instructions given by a clinician. Do not give a child someone else's medicine, and do not delay care while trying home remedies if breathing is difficult.

Arrange medical advice if the barking cough is new, worsening, or accompanied by noisy breathing. A clinician can assess the airway and decide whether treatment is needed.

Seek urgent medical help if the child has noisy breathing while resting, pulls in at the ribs or neck with each breath, cannot speak or cry normally because of breathlessness, has blue or grey lips, is drooling or unable to swallow, becomes very drowsy, or is getting worse quickly. If the child appears severely short of breath or unresponsive, call emergency services immediately.

Tell the clinician when the cough began, whether it is worse at night, and whether noisy breathing occurs when the child is calm.
