Hip trochanter pain is pain over the bony outer side of the hip and may come from irritated tendons, nearby muscles, or the hip joint. It can be worse when lying on that side, climbing stairs, walking uphill, standing on one leg, or rising from a chair. The location helps guide an assessment, but it does not identify one diagnosis by itself.

What the pain may mean

Pain at the greater trochanter—the prominent bone at the outside of the upper thigh—is often linked with irritation of the gluteal tendons or the tissues around them. This pattern is sometimes called greater trochanteric pain syndrome. Gluteal tendinopathy presents with outer-hip pain and tenderness, particularly when the hip is compressed during side-lying or crossed-leg positions. Pain can also be referred from the lower back or come from a problem inside the hip, such as hip osteoarthritis, hip impingement, or a labral tear. A fall or direct blow raises concern for an injury, including a hip fracture, especially when weight-bearing is difficult.

For a mild, recent problem, reduce activities that sharply increase the pain while keeping comfortable movement. Avoid lying on the painful side; if you sleep on the other side, place a pillow between your knees. Choose level walking for now and build activity back gradually. A physiotherapist can check hip and pelvic movement, then prescribe progressive strengthening for the gluteal muscles rather than relying on forceful stretching of a sore area. Ice or heat may provide short-term comfort; protect your skin and stop if either makes symptoms worse. A pharmacist or clinician can help you choose pain relief safely if you have other health conditions or take regular medicines.

When to seek care

Arrange a clinical assessment if the pain is persistent, keeps returning, disturbs sleep, limits work or walking, or does not settle after sensible activity changes. Seek urgent care after a fall or impact if you cannot bear weight, the leg looks shortened or rotated, or pain is severe. Prompt assessment is also needed for a hot, red, swollen hip with fever, new weakness or numbness, or loss of bladder or bowel control. These signs need evaluation because they can indicate a significant injury, infection, nerve problem, or another condition requiring timely treatment.

Ask what movement or finding points to the likely pain source, whether the lower back or the inside of the hip also needs assessment, and which activities should be modified. You can also ask how to progress strengthening, what improvement should look like, and whether imaging is useful in your situation. Is trochanter pain the same as hip joint pain?

Not necessarily. Trochanter pain is usually felt on the outside of the hip, while pain from inside the joint may be felt in the groin or front of the thigh. Symptoms can overlap, so an examination is useful when the source is unclear.

Should you stretch an aching outer hip?

Gentle movement may be helpful, but forceful stretching or positions that compress the outer hip can aggravate irritated tendons. A physiotherapist can tailor mobility and strengthening to your symptoms.

Can back pain be felt at the outer hip?

Yes. Irritation of nerves or joints in the lower back can refer pain toward the buttock and outer hip. Numbness, tingling, or weakness makes a clinical assessment more important.
