What should you know about hip stress fracture surgery?

Short answer
Hip stress fracture surgery may stabilise a vulnerable hip fracture when rest and protected weight-bearing are not enough or the fracture is at risk of displacement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main concern
- A hip stress fracture can worsen with continued loading.
- Assessment
- Imaging and examination guide the treatment choice.
- Recovery focus
- Protect the repair, then rebuild movement and strength gradually.
What hip stress fracture surgery means
A stress fracture is a small crack caused by repeated loading rather than one major injury. Around the hip, its exact location matters: some fractures can heal with activity restriction, while others need prompt stabilisation because walking can make the crack extend. An orthopaedic clinician may use examination and imaging to assess the fracture, bone strength, symptoms, and ability to walk safely.
Surgery commonly involves placing screws or another fixation device to hold the bone in a stable position while it heals. The plan depends on the fracture pattern and whether the joint is affected. Vitamin D deficiency can be one factor considered when assessing bone health; treatment may include clinician-directed supplementation.
What to do before and after treatment
Until you have a clear plan, avoid running, jumping, and putting extra weight through a painful hip. Use crutches or another aid exactly as advised. Do not test the hip by repeatedly walking through increasing pain. Before surgery, ask which medicines and supplements to pause, how to prepare for anaesthesia, and what assistance you will need at home.
After fixation, follow the weight-bearing limit and wound-care instructions. Rehabilitation usually progresses from safe transfers and gentle movement to strength, balance, and a gradual return to walking. A physiotherapist can help adjust activity without overloading the healing bone. Persistent pain, limping, or pain elsewhere such as the thigh, shin, heel, or foot should be reported rather than ignored.
When to seek urgent care
Seek urgent assessment after a fall, sudden increase in hip or groin pain, new inability to bear weight, or a marked change in walking. After surgery, seek prompt help for fever, increasing redness or drainage from the wound, new calf swelling, chest pain, breathlessness, or sudden leg numbness or weakness.
Questions for your doctor
Ask: Where exactly is the fracture, and does its position make surgery advisable? What weight can I safely place on the leg now? Which fixation method is being considered? How will bone health be checked? What symptoms should make me return sooner?

Questions people ask
Does every hip stress fracture need surgery?
No. Some fractures can heal with strict activity modification and protected weight-bearing, while a fracture in a higher-risk location may need fixation.
When can I walk normally after surgery?
Walking progression depends on the fracture, fixation, imaging, pain, and your clinician’s weight-bearing instructions.
