Heavy bleeding 3 weeks after a C-section

Postpartum discharge, called lochia, often changes from red to lighter colours and gradually decreases. Activity can sometimes make flow temporarily heavier, but a return to heavy bright-red bleeding or a sudden increase needs review. Causes may include retained placental tissue, infection or the uterus not contracting as expected; a first period can also return later, particularly when not breastfeeding.

The birth method matters, but symptoms matter more. Tell the maternity team about clots, fever, worsening abdominal pain, wound changes, dizziness and any unpleasant smell.

Contact your maternity unit, obstetric team or urgent care service today for advice about heavy bleeding at this stage. Use pads so the amount and colour can be observed, rest, and avoid inserting anything into the vagina until reviewed. Do not delay because the incision looks normal.

Call emergency services for very heavy flow, faintness, confusion, severe abdominal pain, breathlessness or a racing heartbeat. Urgent same-day assessment is needed for fever, chills, foul-smelling discharge, worsening pain or a wound that is becoming red or draining.

Ask whether the bleeding pattern is within expected postpartum recovery, whether you need examination or imaging, and which symptoms should send you back to hospital.
