Short answer

Heart-related shortness of breath can result from reduced pumping efficiency, an abnormal rhythm or reduced blood flow to heart muscle and needs urgent help when severe or sudden.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How heart-related breathlessness may present

Breathlessness from heart failure may worsen with activity or when lying flat and can occur with ankle swelling, tiredness or a cough. Angina may cause pressure or discomfort with exertion and may be accompanied by breathlessness. Atrial fibrillation can make the heartbeat irregular and reduce exercise tolerance. Anaemia can also leave a person short of breath because less oxygen is carried in the blood. Lung conditions such as asthma, COPD, acute bronchitis or bronchiectasis are other possible explanations. Anaphylaxis can cause sudden breathing difficulty with swelling, wheezing or a widespread reaction. The pattern and examination matter more than the symptom name alone.

What to do now

Stop exertion, sit upright and loosen restrictive clothing. Note whether symptoms began suddenly, occur at rest or follow activity, and record chest discomfort, palpitations, swelling, cough or wheezing. Use a prescribed reliever or heart medicine only as directed. Arrange prompt medical assessment for new, recurring or worsening breathlessness, even if it settles.

When to seek emergency care

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, inability to speak in full sentences, chest pressure, fainting, confusion, or sudden breathlessness with facial or throat swelling. Do not drive yourself. Sit upright while waiting and follow the emergency operator's instructions.

Questions for your doctor

Ask whether your symptoms fit a heart, lung or blood-related cause, whether you need an electrocardiogram, chest assessment or blood tests, and which activities or warning changes should prompt immediate help. Share your medication list and symptom pattern.

Questions people ask

Can breathlessness be caused by something other than the heart?

Yes. Lung conditions, anaemia, allergic reactions, infection and other problems can cause it, so assessment is needed to identify the source.

Why can lying down worsen heart-related breathlessness?

Fluid congestion and changes in pressure when lying flat can make breathing feel harder in some people with heart failure.
