Heart failure and chest pain

Heart failure itself more often causes breathlessness, fatigue and fluid retention than isolated chest pain. Pain or pressure can occur when the heart muscle does not receive enough blood, and an irregular rhythm may cause discomfort with palpitations or weakness. Chest symptoms can also come from the lungs, muscles, reflux or anxiety, so their location and character cannot safely identify the cause at home.

Tell a clinician whether the sensation is pressure, tightness, burning or sharp pain; where it travels; what brings it on; how long it lasts; and whether sweating, nausea, breathlessness or faintness occurs with it. A history, examination and appropriate tests guide the decision.

Stop exertion and sit somewhere safe while you assess symptoms. If pain is new, recurring or linked to activity, obtain prompt medical advice even if it settles. Take prescribed heart medicines as directed, but do not use someone else’s medicine or drive yourself if symptoms are severe.

Keep a note of timing, triggers, associated symptoms and recent medicine changes. This can help the clinician distinguish a stable pattern from a change requiring urgent investigation.

Call emergency services immediately for chest pressure or pain that is severe, persistent, spreading to the arm, back, jaw or neck, or accompanied by sweating, nausea, fainting or marked breathlessness. Sudden weakness, confusion or blue lips also require emergency care. Do not wait for a routine appointment when these signs are present.

Ask what causes are most relevant to your history, whether you need an ECG or other tests, and which symptoms should trigger emergency help. Discuss safe activity, how to use any prescribed rescue medicine, and whether your heart-failure treatment or blood pressure plan needs review.
