What it may mean
An exertion headache can begin during intense activity or soon afterwards as blood flow and pressure change. Exercising in a hot, humid setting, starting suddenly, holding your breath, or not drinking enough can add strain. A headache with thirst, dark urine, cramps, unusual tiredness, or dizziness can occur with dehydration; heat exhaustion may also cause headache alongside heavy sweating, weakness, nausea, or cool clammy skin. Nasal congestion from allergic rhinitis, a common cold, or chronic sinusitis can make exercise feel harder and may contribute to head or facial pressure. COVID-19 and dengue fever can also cause headache with other illness symptoms, so do not train through fever or marked weakness.
What to do now
Stop or reduce the workout, move to a cool place, loosen tight clothing, and rest. Sip water gradually and avoid returning to strenuous activity while the headache or other symptoms continue. Once well, warm up progressively, breathe continuously rather than straining, and avoid exercising in peak heat. Record when the pain starts, its location, intensity, activity, and associated symptoms. A clinician can help assess a new, recurring, or exercise-triggered headache, especially if it began abruptly.
When to seek care
Seek urgent medical help for a sudden severe headache, fainting, confusion, weakness, numbness, trouble speaking, vision loss, chest pain, breathing difficulty, repeated vomiting, stiff neck, or a high fever. Get prompt assessment if the headache follows a head injury, occurs for the first time during exertion, or does not settle with rest. Heat exhaustion symptoms that worsen, or inability to drink, also need urgent attention.
