Headache Behind the Ear: What It May Mean and What to Do

Short answer
A headache behind the ear can arise from neck tension, irritation of nearby nerves, jaw strain, an ear problem, or a general headache disorder; the location alone cannot identify the cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pain location
- Behind one or both ears, sometimes spreading to the scalp or neck
- Useful clues
- Neck movement, chewing, touch, ear symptoms, fever, and neurologic changes
- First steps
- Fluids, rest, gentle movement, and a warm or cool pack
The short answer
Notice whether the pain is on one side or both, dull or electric, and whether it changes when you move your neck, chew, touch the scalp, or lie down. These details help a clinician separate pain referred from the neck or jaw from pain linked to an ear or another headache pattern.
What it may mean
Tight neck and upper-shoulder muscles can refer pain toward the back of the head and behind an ear, especially after prolonged screen use, an awkward sleeping position, or stress. Irritation of an occipital nerve may cause brief stabbing, burning, or shock-like pain that travels over the scalp.
Jaw clenching or a temporomandibular joint problem may cause pain near the ear with clicking, tenderness, or discomfort while chewing. An ear infection or inflammation may be accompanied by earache, reduced hearing, drainage, fever, or pain when the outer ear is touched. Congestion from a common cold, allergic rhinitis, or chronic sinusitis can also create pressure around the head, although behind-ear pain is not specific to these conditions.
Dehydration, heat exhaustion, viral illness, and anxiety-related muscle tension can contribute to headache. A clinician may need to examine your ears, jaw, neck, nerves, and blood pressure to understand the pattern.
What you can do now
Rest in a quiet place, drink fluids, and eat if you have missed a meal. A warm pack over tense neck muscles or a cool pack on the painful area may help; use whichever feels better. Gentle neck movement is reasonable, but stop if it increases the pain. Avoid forceful massage directly over a tender spot or nerve.
Reduce screen strain, check that your pillow keeps your neck neutral, and try to unclench your jaw by keeping your teeth apart when you are not eating. If you use an over-the-counter pain medicine, follow the package directions and check with a pharmacist if you are pregnant, have kidney or stomach problems, take blood thinners, or use other medicines. Repeated pain-reliever use can itself contribute to frequent headaches, so seek advice if you need it often.
When to seek care
Get emergency help for a sudden, extremely severe headache, or headache with weakness, numbness, confusion, fainting, a seizure, trouble speaking, or new vision loss. Seek urgent assessment for fever with a stiff neck, a spreading rash, repeated vomiting, severe dizziness, or a severe headache after a head injury.
Arrange a medical review if the pain keeps returning, lasts longer than expected, is becoming more severe, or comes with ear discharge, hearing change, jaw difficulty, scalp tenderness, or neck pain. People with a weakened immune system, pregnancy, or a new headache later in adulthood should seek advice promptly when the pattern is new or unusual.
Questions for your doctor
You could ask: Could this pain be coming from my neck, jaw, ear, or a nerve? What findings would make an ear examination, dental review, or further testing useful? Which pain medicine is suitable with my health conditions and current medicines? What symptoms should prompt urgent care if the headache returns?
