Headache with throwing up can have several causes, so the pattern and warning signs matter more than the symptom alone. Migraine is one possibility, while viral illness, dehydration, heat exhaustion, sinus symptoms, anxiety, or another medical problem may also be involved.

What this combination can mean

A migraine may cause throbbing head pain, nausea, vomiting, and sensitivity to light or sound. A cold, COVID-19, or sinus condition may bring headache alongside congestion, facial pressure, sore throat, cough, or fever. Heavy sweating, prolonged heat exposure, or poor fluid intake can contribute to dehydration or heat exhaustion; dizziness, weakness, thirst, and dark urine can occur with fluid loss. Dengue fever can also cause severe headache with fever, body aches, nausea, or vomiting, especially after exposure in an area where dengue occurs.

The symptom does not identify its cause by itself. A clinician may ask when the pain began, where it is located, whether it was sudden, what other symptoms appeared, and whether you can keep fluids down.

What you can do now

Rest in a quiet, cool place and take small, frequent sips of water or an oral rehydration drink. If vomiting settles, try bland foods in small amounts and avoid alcohol, intense exercise, and foods that clearly worsen nausea. Reduce bright light and strong smells if they aggravate the headache. Check your temperature and note the timing of pain, vomiting, fluid intake, medicines, and any recent heat or travel exposure.

Do not take more pain medicine than the label allows, and avoid aspirin or ibuprofen if a clinician has advised against them, if you may have dengue, or if you have a condition that makes them unsafe. Arrange medical advice if symptoms keep returning, are worsening, or do not improve after hydration and rest.

When to seek urgent care

Seek emergency help for a sudden, extremely severe headache; new weakness, numbness, confusion, fainting, trouble speaking, or a seizure; a stiff neck with fever; repeated vomiting with inability to keep fluids down; severe drowsiness; vision loss; or a headache after a head injury. Urgent assessment is also needed for signs of serious dehydration, blood in vomit, black stools, severe abdominal pain, breathing difficulty, or a rapidly worsening condition.

Children, older adults, pregnant people, and anyone with an immune-system condition or significant long-term illness should seek medical advice sooner when headache and vomiting occur.
