Head psoriasis: causes and next steps

Psoriasis involves an overactive immune response that speeds the life cycle of skin cells. The result can be clearly bordered, red or darker-than-usual patches covered by dry, silvery, or thick scale. It can extend beyond the hairline and may itch, feel sore, crack, or shed flakes. A flare can follow scalp injury, stress, illness, cold dry weather, or irritation from products, although a trigger is not always found. Scalp psoriasis is not caused by being unclean and is not spread by contact. Other conditions, including dermatitis or a fungal infection, can look similar, so appearance alone cannot confirm the cause.

Arrange an assessment if scaling is new, persistent, or difficult to distinguish from another rash. Use prescribed scalp medicine as directed and allow it to reach the skin rather than only coating the hair. Loosen scale gently; picking or forceful scraping can cause bleeding and may worsen plaques. Choose fragrance-free products if the scalp is sensitive, and pause any product that causes burning or a new rash. Keep nails short to limit injury from scratching. Tell the clinician about joint stiffness or swollen fingers, because psoriasis can involve more than the skin.

Seek medical review for painful cracking, bleeding, pus, warmth, rapidly spreading redness, or fever. Book an appointment when hair loss occurs in patches, the scalp becomes tender, or treatment stops helping. New joint pain, prolonged morning stiffness, or swollen joints also merits assessment. Urgent help is needed for a widespread blistering or peeling reaction, facial swelling, or breathing difficulty after a medicine.
