Hard stool after delivery: what to know

Short answer
Hard stool after delivery is often related to slower bowel movement, reduced fluids, pain, or medicines, and gentle treatment can help.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common contributors
- Pain, reduced movement, low fluid intake, iron, and some pain medicines
- Helpful habit
- Avoid straining and give yourself time when you feel the urge
Why it can happen
After birth, soreness around the perineum, fear of straining, a caesarean recovery, reduced activity, and changes in eating or drinking can make bowel movements harder. Iron supplements and some prescription pain medicines may also slow the bowel. Passing stool should not require force; straining can worsen discomfort or put pressure on stitches or haemorrhoids.
Practical relief
Drink regularly unless a clinician has restricted your fluids. Add fibre gradually through fruit, vegetables, whole grains, beans, or similar foods, and take short gentle walks when safe. Respond to the urge to open your bowels, use a footstool to raise your knees, and breathe out rather than holding your breath. A pharmacist or maternity clinician can advise on a suitable stool softener or laxative, including options compatible with breastfeeding.
When to seek care
Contact your maternity team if constipation does not improve, keeps recurring, or makes you avoid eating or drinking. Seek urgent help for severe or worsening abdominal pain, repeated vomiting, a swollen abdomen, inability to pass stool or gas, heavy bleeding, fever, or faintness. Painful bowel movements with a new lump or significant rectal bleeding also deserve assessment.
Questions for your doctor
Ask whether an iron or pain medicine could be contributing, which bowel treatment is suitable while breastfeeding, and whether your stitches or haemorrhoids need examination.

Questions people ask
Can hard stool damage delivery stitches?
Straining can increase discomfort and pressure, so tell your maternity clinician if bowel movements are painful or difficult.
What if I am breastfeeding?
Ask a pharmacist or clinician to recommend an option appropriate for breastfeeding rather than starting a product without advice.
