A ground-glass appearance in an ovarian cyst describes fine internal echoes on ultrasound, often associated with blood products such as an endometrioma. It is an imaging description, not a diagnosis by itself. The cyst’s size, shape, blood flow, symptoms, age, pregnancy plans, and changes over time help determine whether monitoring, further imaging, or treatment is appropriate.

The phrase means the fluid inside the cyst looks relatively uniform and hazy rather than completely clear. This pattern can occur when old blood is present, including with an ovarian endometrioma, but ultrasound findings can overlap. A clinician may compare the scan with earlier images, assess both ovaries, and consider symptoms such as pelvic pain, painful periods, pain during sex, or difficulty becoming pregnant. The appearance alone does not establish cancer.

Ask the clinician who arranged the scan what the report recommends and whether a repeat transvaginal ultrasound is needed. Keep a record of pelvic pain, bleeding pattern, bowel or bladder symptoms, and medicines. Management may involve observation when the cyst is uncomplicated and symptoms are limited. Hormonal treatment may help some endometriosis-related symptoms but does not suit everyone. Surgery can be discussed when a cyst is persistent, large, painful, or has features requiring clarification.

Seek urgent help for sudden severe one-sided pelvic pain, fainting, shoulder-tip pain, fever, repeated vomiting, or heavy bleeding. These symptoms can occur with complications such as twisting or rupture and need prompt assessment.
