What it can feel like

The bursa cushions tissues near the greater trochanter, the bony prominence at the side of the hip. Pain may spread down the outer thigh and become more noticeable when lying on that side, climbing stairs, standing from a chair, or walking uphill. Nearby gluteal tendons can also be irritated, so a clinician may use the broader term greater trochanteric pain syndrome.

A physical examination helps distinguish this pattern from pain coming from the hip joint, lower back, or another structure. Visible swelling is not always present; tenderness over the side of the hip can be more useful.

Reduce activities that repeatedly compress the painful side, but keep gentle movement within comfortable limits. Avoid lying directly on the sore hip; a pillow between the knees may reduce pressure when lying on the other side. Ice wrapped in a cloth can be used briefly after aggravating activity.

A clinician may discuss an anti-inflammatory medicine when it is appropriate for your health history. Physiotherapy can address hip and pelvic strength, walking mechanics, and gradual return to activity. Persistent symptoms may lead to discussion of a cortisone injection; joint aspiration is considered only when fluid needs assessment or removal.

Arrange an assessment if pain persists, interferes with sleep or walking, or returns whenever normal activity resumes. Seek urgent medical care for a hot, red, rapidly swollen area, fever, severe pain after a fall, inability to bear weight, or new leg weakness or numbness.
