What should you know about gout pseudogout difference?

Gout occurs when monosodium urate crystals collect in or around a joint. Pseudogout, or calcium pyrophosphate deposition disease, involves calcium pyrophosphate crystals. The immune response to either crystal can cause a sudden flare with pain, warmth, redness and joint swelling.

The affected joint may offer clues but cannot confirm the diagnosis. Gout often involves the big toe and may affect the ankle or knee. Pseudogout often affects a larger joint such as the knee or wrist. These patterns overlap, and an infection can cause similar symptoms, so a hot swollen joint deserves proper assessment.

A clinician may review your symptoms, medicines, health conditions and previous attacks. Joint aspiration can collect fluid for crystal examination and help exclude infection. Imaging may show deposits or joint changes, but it does not always identify the cause of a sudden flare by itself.

Management depends on the crystal type and your health. Anti-inflammatory medication may be considered for either condition when suitable. A cortisone injection may be an option for selected joints after infection has been considered. Gout may also need a separate long-term plan to lower urate; that approach does not treat pseudogout crystals.

Seek urgent care for a swollen joint accompanied by fever, chills, confusion, severe weakness or rapidly increasing pain. Contact a clinician promptly for a first attack, a flare after an injury, or symptoms that are not improving with the agreed plan.

Ask whether the joint should be aspirated, how infection will be ruled out, which crystal is most likely, and whether your medicines or kidney function change the treatment choices.
