Short answer

Gout in the small toe can cause sudden joint pain, warmth, redness and swelling, and needs assessment when symptoms are new or severe.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical finding
Sudden tenderness, warmth, redness and swelling around the small-toe joint.
During a flare
Protect the toe, use cool packs briefly and avoid tight footwear.

What it may mean

Gout is an inflammatory arthritis caused by urate crystals collecting in a joint. Although the big toe is a familiar site, the small toe can also be affected. A flare may begin abruptly, with skin that feels hot and a joint that is very sensitive to touch or footwear. Similar symptoms can come from an injury, infection or another form of arthritis, so appearance alone cannot confirm gout. A clinician may examine the toe, review medicines and health conditions, and use blood tests or joint fluid testing when needed.

What you can do

Rest the foot and keep the small toe raised when practical. Use a cool pack wrapped in cloth for short periods, and choose roomy footwear that does not press on the joint. Drink normally unless a clinician has restricted your fluids. Do not start, stop or change gout medicine without medical advice; treatment choices can depend on kidney function, other medicines and whether this is an acute flare or prevention plan. After the flare, discuss ways to reduce future attacks, including reviewing triggers and long-term urate management.

When to seek care

Arrange prompt medical care for a first episode, rapidly worsening swelling, marked redness, fever, chills, a wound, or difficulty walking. An infected joint can resemble gout and requires urgent treatment. Seek emergency help if you feel severely unwell or the pain follows a significant injury. People with chronic kidney disease, diabetes or immune problems should use a lower threshold for assessment.

Questions for your doctor

Ask whether the small-toe findings fit gout or another cause, whether testing is needed, which pain medicines are suitable with your health history, and how to prevent another flare. Mention every prescription, supplement and over-the-counter medicine.

Yes. Gout can affect joints beyond the big toe, including a small-toe joint.

Yes. A first or unusually severe episode should be assessed because infection and injury can look similar.
