How gout can involve these joints

Urate crystals can collect in a joint and trigger inflammation. In the hands and wrists, this may cause pain with gripping, typing or turning a key, along with warmth, redness and reduced movement. A flare can affect one area or several joints, and stiffness may linger after the acute pain improves. Firm deposits can appear near joints after ongoing disease. These signs do not prove gout: infection, trauma and other inflammatory conditions can also cause a swollen wrist or hand. Clinical assessment is especially useful when the symptoms are new, severe or different from previous attacks.

Managing symptoms safely

Rest the hand and wrist in a neutral, supported position, and raise them when sitting. Remove rings and tight accessories. Apply a cold pack wrapped in cloth for short intervals, allowing the skin to return to normal between applications. Avoid forceful stretching, heavy lifting and deep massage during intense inflammation. Follow any prescribed flare instructions, and do not change preventive gout treatment without advice. Tell your clinician about Chronic Kidney Disease or other medicines because these can affect which options are suitable. Once pain settles, gentle movement can help prevent unnecessary stiffness if it does not increase symptoms.

Urgent and routine review

Seek urgent assessment for a hot swollen wrist or hand with fever, chills, a wound, pus, spreading redness or a rapidly worsening illness. Go promptly if the hand becomes numb, pale, blue or unusable. Arrange routine review for a first suspected flare, repeated episodes, lasting swelling, hard deposits or ongoing loss of grip. Infection can damage a joint, so a red hot joint should not be labelled gout without appropriate consideration.
