Getting pregnant with hydrosalpinx

Hydrosalpinx happens when a fallopian tube becomes blocked and fills with fluid. The egg and sperm may be unable to meet through that tube. If the other tube is open and functioning, natural pregnancy can still occur, although fertility depends on ovulation, sperm health, age and other pelvic factors. Previous pelvic infection, including Pelvic Inflammatory Disease, can damage the tubes.

The fluid may also flow back towards the womb, which can make the womb environment less suitable for an embryo. Your clinician may use ultrasound or a dye test to assess the tubes and discuss whether treatment should happen before assisted reproduction.

Arrange a fertility assessment rather than relying only on cycle tracking. The assessment may include an ovulation review, semen analysis and imaging of the womb and tubes. Ask whether the affected tube should be treated or removed before trying naturally or proceeding with IVF.

Treatment is individual. A salpingectomy removes a fallopian tube and may be considered when hydrosalpinx is affecting fertility planning. IVF bypasses the fallopian tubes by placing an embryo in the womb; your fertility team can explain timing, alternatives and how the remaining tube affects your options.

Seek urgent medical help for severe one-sided pelvic pain, fainting, shoulder-tip pain, heavy bleeding or fever, especially after a positive pregnancy test. These symptoms can signal an ectopic pregnancy, infection or internal bleeding. Contact a fertility or gynaecology service promptly if you have known hydrosalpinx and are trying to conceive, because tubal disease needs a tailored plan.

Ask whether one or both tubes are affected, whether the fluid is changing your treatment plan, which tests are needed, and whether surgery or IVF should come first. Also ask how long to try naturally, how pregnancy will be monitored, and what symptoms require immediate assessment.
