What should you know about treadmill gait analysis?

Short answer

Treadmill gait analysis lets a clinician observe walking or running at controlled speeds while assessing movement, loading, and symptom response.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What happens on the treadmill

You may begin at an easy pace while the clinician observes your stride, cadence, foot contact, knee and hip control, balance, and trunk position. Speed or incline can be adjusted when appropriate to see whether a pattern changes with effort. The treadmill offers repeatable conditions, but it may not perfectly represent outdoor running, uneven ground, turns, or sport-specific movements. Your report should therefore be read with your history and examination, not as a stand-alone diagnosis.

Cameras may provide a simple visual record. [3D Motion Capture](/technology/3d-motion-capture/) can quantify movement, and a [Pressure Plate Gait Scanner](/technology/pressure-plate-gait-scanner/) can add information about pressure beneath the feet.

Before and after the assessment

Bring the shoes you normally use and explain your usual routes, pace, recent training changes, and exact symptom location. Tell the clinician promptly if discomfort, dizziness, or unsteadiness develops. Findings may lead to exercises for strength or control, a temporary training modification, footwear advice, or [Custom Orthotics](/treatments/non-surgical-orthopedics/custom-orthotics/). Make one change at a time when possible so you can tell how your body responds.

When not to wait for a gait test

Seek care promptly for severe pain after an injury, inability to walk safely, a new foot drop, rapidly increasing swelling, or numbness with weakness. A treadmill assessment is generally planned for a stable concern; urgent symptoms need clinical evaluation first. Stop the test if your symptoms become sharp, progressive, or associated with feeling faint.

Useful questions

Ask what speed and conditions will be tested, whether your usual shoes should be worn, and which findings are meaningful for your complaint. Clarify the recommended exercise or training change, how to scale it, and which symptoms should prompt a review.

Questions people ask

Is treadmill gait analysis the same as running outdoors?

No. It provides controlled observations, while outdoor surfaces, turns, and pace changes may produce different demands.

Should I run through pain during testing?

No. Report symptoms as they arise so the clinician can adjust or stop the assessment safely.
