What should you know about forefoot bursitis?

Short answer
Forefoot bursitis is irritation of a fluid-filled cushion beneath the front of the foot, often causing local tenderness and pain with walking.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Focal tenderness with pressure
What it means
A bursa reduces rubbing between tissues. In the forefoot, repeated pressure from narrow footwear, prolonged standing, running, altered foot mechanics, or a prominent joint can irritate this cushion. You may notice a focused tender spot, swelling, warmth, or a bruised feeling beneath the ball of the foot. Similar symptoms can come from a stress injury, nerve irritation, arthritis, or a skin problem, so an examination matters.
What you can do
Reduce activities that reproduce the pain and choose shoes with a broad toe box, soft uppers, and room for the forefoot. A cushioned insert or metatarsal pad may redistribute pressure when fitted correctly; it should not create more pain. Elevating the foot after activity and using a wrapped cold pack for short periods may settle irritation. A clinician may discuss anti-inflammatory medication, a cortisone injection, or joint aspiration when examination findings support those options.
When to seek care
Arrange an assessment if pain persists despite reducing pressure, returns repeatedly, or limits normal walking. Seek prompt care for rapidly increasing swelling, marked redness or warmth, drainage, fever, a sudden inability to bear weight, or numbness and colour change in the toes. People with diabetes, reduced circulation, or reduced sensation should seek advice early for new foot swelling or wounds.
Questions for your doctor
Ask which structure is painful, whether imaging is needed, how to adjust footwear, and whether a pad or treatment could affect your walking pattern. Ask when you can safely return to work, exercise, or sport.
