Flank pain while urinating: what to know

Burning, frequent urination, urgency, cloudy urine, or lower abdominal discomfort can occur with a urinary tract infection. When infection reaches the kidney, flank pain may occur with fever, chills, nausea, or feeling very unwell. Kidney stones can cause sharp or cramping flank pain, blood in the urine, and nausea; pain may travel toward the groin. The timing of pain during urination does not identify the cause by itself. A clinician may use symptoms, an examination, urine testing, and sometimes imaging to distinguish infection, a stone, or another source.

Arrange prompt medical advice if flank pain accompanies burning, urgency, blood in the urine, or repeated urination. Note the pain's side, severity, pattern, and whether urine output has changed. Drink normally unless you have been told to restrict fluids, and avoid leftover antibiotics because the correct treatment depends on the cause. A clinician may request a urine sample before treatment and decide whether further assessment is needed.

Seek urgent care for severe or escalating pain, fever or chills, repeated vomiting, confusion, faintness, inability to urinate, or visible blood in the urine. Pregnancy, one functioning kidney, known kidney disease, or a weakened immune system lowers the threshold for prompt assessment. These features can signal infection or obstruction needing timely treatment.

Ask whether you need urine culture or imaging, how to manage pain safely, and which symptoms should bring you back urgently. Tell the clinician about pregnancy, allergies, kidney problems, recent antibiotics, and any history of stones or urinary infections.
