What should you know about flank pain after ejaculation?

Sexual activity can tighten the pelvic floor, abdominal wall, and lower back, producing soreness that changes with posture or muscle use. Pain during or after ejaculation may also occur with irritation or inflammation in the urinary or reproductive tract. Burning urination, discharge, fever, pelvic pain, blood in urine or semen, or testicular swelling should not be explained away as muscle tension. Flank pain that is colicky, severe, or unrelated to movement may need evaluation for a kidney stone or other urinary cause.

Pause sexual activity and strenuous exercise until the pain settles. Gentle walking, relaxed breathing, and warmth over a sore muscle may be more helpful than forceful stretching. Drink fluids normally if you have no restriction, and avoid irritants that clearly worsen urinary discomfort. Do not take someone else’s antibiotics or conceal symptoms from a clinician because they feel embarrassing. Note whether the pain follows ejaculation every time, how long it lasts, and whether urination, erections, discharge, or bowel movements change it.

Arrange prompt assessment for repeated or persistent post-ejaculatory pain, painful urination, discharge, fever, blood, or pelvic tenderness. Seek urgent help for sudden severe testicular pain or swelling, severe flank pain with vomiting, fainting, inability to urinate, or fever with shaking chills. These symptoms can need rapid treatment.

Ask whether you need an abdominal, pelvic, genital, or prostate examination and urine testing. Tell the clinician about new partners, protection used, urinary symptoms, medicines, prior stones or infections, and the exact relationship between ejaculation and pain.
