Extreme fatigue during pregnancy: what to know

Short answer
Extreme fatigue during pregnancy can be common, but severe or worsening tiredness should be discussed because anaemia, dehydration, infection, or another condition may need treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common contributors
- Hormonal change, poor sleep, nausea, and increased physical demand.
- Check sooner
- Severe, worsening, or function-limiting fatigue.
Why pregnancy can leave you exhausted
Hormonal shifts, a changing sleep pattern, nausea, increased physical demands, and emotional load can all lower energy. Fatigue may be stronger early on and can return later as sleep becomes less comfortable. Its intensity does not by itself show how pregnancy is progressing.
Anaemia is a frequent possibility to check, particularly with breathlessness, paleness, headaches, or a racing heartbeat. Dehydration may bring thirst, dizziness, dry mouth, and darker urine. Fever, cough, diarrhoea, or painful urination points more towards infection and warrants medical advice.
Ways to conserve energy
Rest when your body signals a need, divide chores into smaller tasks, and accept practical help. Eat regular balanced meals, keep water nearby, and rise slowly if you feel dizzy. Gentle movement may help some people, but stop if it worsens symptoms. Continue only pregnancy-safe medicines and supplements recommended by your maternity clinician.
Mention fatigue at antenatal care, even if it seems expected. The clinician can review your blood results, nutrition, sleep, mood, medicines, and any pregnancy-specific concerns.
Get help promptly for warning signs
Seek urgent care for fainting, chest pain, severe breathing difficulty, confusion, sudden weakness, severe headache with visual changes, heavy vaginal bleeding, severe abdominal pain, or markedly reduced urination. Contact your maternity team promptly for persistent vomiting, fever, or fatigue that prevents drinking, eating, walking safely, or caring for yourself.
Useful questions at an appointment
Ask whether your blood count and iron status are suitable, whether your fluid intake is enough, and whether another test or review is needed. Tell them when fatigue began, how it affects daily life, and whether sleep, mood, bleeding, pain, or illness has changed.

Questions people ask
Is extreme tiredness always normal in pregnancy?
No. It can be expected, but marked or changing fatigue can also reflect a treatable problem.
Can I take an iron supplement for fatigue?
Ask your maternity clinician first; testing helps identify whether iron is appropriate.
