What extreme fatigue may mean

Pregnancy changes hormones, circulation, sleep, appetite, and the body's energy demands. Nausea, vomiting, emotional strain, disrupted sleep, or carrying more than one pregnancy can make tiredness harder to manage. Fatigue may also accompany anaemia, coeliac disease, COVID-19, or dehydration. Chronic kidney disease and other long-term conditions can add to low energy. The symptom alone cannot identify a cause.

Notice whether the tiredness began suddenly, limits ordinary activities, follows vomiting or poor fluid intake, or comes with fever, dizziness, paleness, palpitations, weight change, or reduced urination. These details help a clinician decide whether examination or blood and urine tests are appropriate.

What you can do now

Rest when possible and lower non-essential demands. Eat regular, nourishing meals and keep fluids available, particularly if nausea is affecting intake. Gentle movement may help some people, but stop if it worsens dizziness, weakness, pain, or breathlessness. Do not start iron, herbal products, or other supplements without checking that they are suitable in pregnancy.

Arrange prenatal care and mention fatigue that is extreme, worsening, or interfering with washing, eating, work, or sleep. Keep a brief record of sleep, food and fluid intake, vomiting, bleeding, fever, and medicines.

When to seek urgent help

Seek urgent medical help for fainting, confusion, severe weakness, chest pain, difficulty breathing, heavy vaginal bleeding, severe abdominal pain, inability to keep fluids down, or very little urine. Contact a maternity service promptly for new symptoms that feel serious or rapidly worsen. Pregnancy does not make every episode of tiredness harmless.
