Extreme chills without fever: what to know

Shivering is the body’s way of generating heat, and it can follow a cold environment, wet clothing, fear, or a drop in blood sugar. An infection may begin before a fever is measurable, or a fever may have eased after medicine. Influenza, pneumonia, urinary tract or kidney infection, malaria, and dengue fever can have different symptom patterns. Chills with confusion, extreme weakness, or rapid breathing raise concern for sepsis and need urgent attention.

Move to a warm, dry place, add comfortable layers, and check your temperature with a reliable thermometer. Drink fluids if you can, eat if you may have missed a meal, and record other symptoms and recent travel. Do not use leftover antibiotics or antimalarial medicines. Arrange a medical review when chills recur, follow travel to a malaria area, or occur with cough, urinary symptoms, rash, vomiting, or pain.

Seek emergency care for confusion, difficulty breathing, fainting, severe weakness, blue or mottled skin, a stiff neck, seizure, or rapidly worsening illness. Get prompt assessment for shaking chills with chest pain, severe headache, persistent vomiting, flank pain, painful urination, or recent travel to an area where malaria or dengue occurs. A normal temperature does not rule out a significant infection.

Ask whether your symptoms fit a possible infection or another trigger, whether tests are needed, and which changes should prompt emergency care. Tell the clinician about travel, insect bites, medicines, diabetes, urinary symptoms, cough, rash, and the timing of temperature readings.
