Extreme bloating: what to know

Short answer
Extreme bloating can result from trapped gas, constipation, food intolerance, or a digestive condition, and persistent, painful, or sudden swelling needs medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Useful diary
- Meals, timing, bowel movements, pain, and associated symptoms
- Do not ignore
- Sudden swelling with severe pain, vomiting, or inability to pass gas
What bloating can reflect
Bloating is a feeling of fullness or visible abdominal distension. Gas, slower movement of the bowel, constipation, and swallowing air can all contribute. Some people notice a clear connection with meals, while others experience symptoms that vary with bowel movements or stress.
Food Intolerance, Lactose Intolerance, Coeliac Disease, Irritable Bowel Syndrome, Helicobacter Pylori Infection, and Giardiasis can present with bloating. Bloating does not identify one cause by itself; accompanying diarrhoea, constipation, pain, fatigue, or weight change helps guide assessment.
What may ease symptoms
Eat at a comfortable pace, choose smaller portions, drink water, and take gentle movement if you are able. Keep a diary of foods, bowel movements, pain, and timing. Avoid removing many foods at once, since that can make triggers difficult to identify and nutrition harder to maintain.
A clinician or registered dietitian can help test a suspected food pattern safely. Nutrition Counselling may be useful when bloating is persistent or your diet has become restricted.
When to seek care
Seek urgent help for sudden severe pain, a hard or rapidly enlarging abdomen, repeated vomiting, inability to pass stool or gas, blood in stool, black stool, fainting, or breathing difficulty.
Arrange a medical appointment for bloating that keeps returning, is progressively worsening, wakes you, or comes with persistent diarrhoea, constipation, fever, vomiting, unintentional weight loss, or marked tiredness.
Questions for your doctor
Ask which causes fit your timing and bowel pattern, whether tests are needed, and how to manage constipation or suspected intolerance. Ask before starting an exclusion diet, and take your food and symptom record to the appointment.
