What should you know about a goitre?

The thyroid sits low at the front of the neck and makes hormones that influence many body processes. Enlargement can be smooth or lumpy, and it may affect one area or the whole gland. Causes include autoimmune thyroid disease, inflammation, some medicines and inadequate iodine intake. A goitre may cause no discomfort, but a larger swelling can create a tight feeling, cough, hoarse voice or difficulty swallowing. Overactive thyroid function can bring heat intolerance, tremor or a racing heartbeat; underactive function may bring low energy, constipation or feeling cold.

Arrange a GP consultation for a new or changing neck swelling. The clinician may examine the gland, ask about medicines and family history, and request a thyroid function test. An ultrasound or other investigation may be suggested when the swelling is uneven or a nodule needs closer assessment. Do not take iodine or thyroid supplements on your own, because extra iodine can alter thyroid activity in some people.

Seek urgent care if neck swelling causes difficulty breathing, inability to swallow liquids, rapidly worsening pressure or a new severe voice change. Arrange prompt assessment for a firm lump, persistent hoarseness, unexplained neck discomfort or growth over a short period. A goitre does not by itself show whether thyroid function is high, low or normal, so symptoms need clinical assessment rather than guesswork.
