Excessive thirst near the end of life

A person who is very unwell may drink less, breathe through the mouth, have a fever, or take medicines that dry the mouth. Dehydration, kidney changes and diabetes can also contribute. Thirst does not by itself show that death is imminent, and it cannot reliably predict how much time remains.

Near the end of life, the body may process fluids differently. Extra drinks or IV fluids can sometimes cause swelling, chest congestion or discomfort, so the care team balances possible benefits with the person’s goals and ability to swallow.

Offer small sips only when the person is awake, willing and able to swallow safely. Moisten the lips and mouth with a damp sponge or soft cloth, provide lip balm, and ask the palliative team about mouth care. Never force drinks into a drowsy person. The team can review medicines, check for treatable causes and discuss whether IV Fluid Therapy fits the care plan.

Call the hospice, palliative or medical team if thirst is new, distressing or accompanied by pain, agitation, vomiting, fever, choking, very little urine or breathing difficulty. Get emergency help for sudden collapse or severe breathing trouble when this matches the person’s agreed goals of care.
