Every time I cough, I pee a little: what should you know?

A cough briefly pushes down on the bladder. If the pelvic-floor muscles and the bladder outlet do not provide enough support at that moment, a small amount of urine may escape. Pregnancy, childbirth, pelvic surgery, constipation, ageing, or a long-lasting cough can contribute. Urgency, burning, pelvic pain, or leakage without coughing may point to a different or additional urinary issue.

Keep a short bladder and cough diary: record drinks, toilet visits, cough triggers, urgency, and leakage. Do not severely restrict fluids, because concentrated urine can irritate the bladder. Prevent constipation, and consider pelvic-floor training with professional guidance. Tightening the pelvic floor just before coughing may help. Arrange an appointment if the leakage is bothersome, increasing, or affecting work, exercise, sleep, or confidence.

Contact a clinician promptly for blood in urine, fever, painful urination, new back or side pain, inability to pass urine, or sudden loss of bladder control with weakness or numbness. Seek urgent care for serious breathing trouble, chest pain, confusion, blue lips, or coughing blood. The cough and the urine leak may require separate evaluations.
