What should you know about etiology of osteoporosis?

Short answer
Osteoporosis develops when bone breakdown outpaces bone rebuilding, leaving bones less dense and more vulnerable to fractures.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why bones lose strength
Bone is living tissue that is continually renewed. Osteoporosis can develop when this renewal becomes unbalanced. Ageing, hormonal changes, a family history, low body weight, limited weight-bearing activity, smoking, heavy alcohol use and low intake or absorption of calcium or vitamin D may contribute. Some medicines and conditions also affect bone formation or mineral balance. A risk factor does not confirm osteoporosis, and a person can have weakened bones without pain or obvious warning signs.
A small fall can cause a hip fracture, vertebral compression fracture or another break when bone strength is reduced. A stress fracture may also develop from repeated loading. A bone-density scan helps estimate bone density and supports treatment decisions, alongside medical history and fracture assessment.
Protecting bone health
Ask a clinician to review your fracture risk, medicines, nutrition, balance and exercise pattern. Weight-bearing and resistance activity can support bone and muscle, while balance work may reduce falls; begin safely if you have pain or a previous fracture. Make the home environment easier to navigate and address vision or dizziness. Supplements are not automatically appropriate: vitamin D supplementation and calcium supplementation should match your needs. Bisphosphonate therapy is one possible prescription option for selected patients.
When to seek care
Seek urgent assessment after a fall or sudden back, hip or limb pain, especially if you cannot stand, walk or use the affected area. New severe back pain, height loss or a stooped posture also deserves prompt review because a vertebral fracture can occur without a dramatic injury. Report side effects from prescribed bone treatment and seek immediate help for severe allergic symptoms or breathing difficulty.
Questions for your doctor
Ask whether you need a bone-density scan, which causes or medicines may be affecting your bones, how to exercise safely, and whether vitamin D, calcium or prescription treatment is suitable for you.

Questions people ask
Does osteoporosis always cause pain?
No. It can remain unnoticed until a fracture occurs, so risk assessment matters even without symptoms.
Can exercise help when bones are fragile?
Appropriate weight-bearing, resistance and balance exercises may help, but the plan should reflect fracture history and fitness.
