What should you know about ESBL pneumonia?

Short answer
ESBL pneumonia is a serious lung infection caused by bacteria that produce enzymes able to resist several antibiotics, so testing guides treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Meaning
- A pneumonia-causing bacterium with resistance to several antibiotics
- Key test
- A respiratory sample with antibiotic sensitivity testing
- Urgency
- Breathing difficulty or sudden confusion needs emergency help
What ESBL means
ESBL stands for extended-spectrum beta-lactamase. These enzymes can break down several penicillin-type and cephalosporin antibiotics. The result is not that every antibiotic will fail; it means the care team needs a sample, often sputum or another respiratory specimen, to identify the organism and its sensitivities.
Pneumonia can cause cough, fever, chills, breathlessness, chest discomfort and coughing up phlegm. Pneumonia is a common presentation of a lung infection, but these symptoms can also occur with influenza or COVID-19, so examination and testing matter.
What to do next
Arrange prompt medical assessment if ESBL pneumonia is suspected or confirmed. A clinician may check oxygen levels, listen to your chest, request imaging and collect samples before choosing an antibiotic when this is safe. Take the prescribed medicine exactly as directed and do not use leftover antibiotics.
Rest, drink fluids if you have not been told to restrict them, avoid smoking, and keep follow-up appointments. Tell the team about recent antibiotics, hospital stays, allergies and any resistant infection previously found. Improvement is monitored through breathing, temperature, oxygen levels and overall strength.
When to seek urgent care
Seek emergency help for severe difficulty breathing, blue or grey lips, new confusion, fainting, inability to stay awake or rapidly worsening weakness. Urgent assessment is also needed if fever persists with worsening cough, chest pain or reduced ability to drink.
Sepsis can develop when an infection triggers a dangerous whole-body response. A racing heartbeat, clammy skin, confusion, very little urine or sudden deterioration needs immediate medical attention.
Questions for your doctor
Ask which organism was identified, which antibiotics it is sensitive to, how long treatment is expected to continue, and whether repeat imaging or a sample is needed. Ask what breathing changes should prompt a return visit and whether close contacts require any specific precautions.

Questions people ask
Can ESBL pneumonia be treated?
Yes. Treatment is selected from the antibiotics the identified bacterium is sensitive to, with monitoring of breathing and clinical recovery.
Is ESBL pneumonia contagious?
Spread depends on the organism and setting. Hand hygiene and following the healthcare team’s instructions reduce transmission risk.
