Effects of goitre in pregnancy

A goitre is an enlarged thyroid gland at the front of the neck. Enlargement does not reveal how much hormone the gland makes. Some people have symptoms of Hypothyroidism, such as unusual tiredness, feeling cold or constipation; others have signs of Hyperthyroidism, such as heat intolerance, tremor or a racing heartbeat. A large swelling can also cause pressure, a tight collar sensation, difficulty swallowing or voice change. Pregnancy alters thyroid hormone needs, making clinical review more useful than judging the swelling by appearance alone.

Tell your antenatal team about a new or growing neck swelling, and bring any previous thyroid results or medicine list. Take thyroid medicine exactly as prescribed and do not add iodine products, supplements or herbal remedies without checking first. Keep scheduled pregnancy appointments so symptoms, blood results and the baby's wellbeing can be considered together.

Contact your maternity team or seek urgent care for severe breathlessness, inability to swallow fluids, rapidly increasing neck swelling, fainting, chest pain, or a sustained racing heartbeat with dizziness. New confusion, severe weakness or repeated vomiting also warrants prompt assessment. These symptoms may reflect pressure from the swelling or a significant thyroid imbalance rather than an ordinary pregnancy change.

Ask what may have caused the swelling, which thyroid results matter in pregnancy, whether medication is needed, when testing should be repeated, and which symptoms should prompt same-day advice.
