Heavy bleeding and ectopic pregnancy

Bleeding in early pregnancy has several possible causes, including miscarriage, cervical bleeding or an ectopic pregnancy. An ectopic pregnancy presents with vaginal bleeding in some people, and the bleeding may be lighter, darker or different from a usual period. The amount of blood alone cannot show the cause. Pain low in the abdomen, particularly on one side, can be a warning that tissue near the pregnancy is being stretched or damaged.

If you are soaking pads quickly, passing clots with worsening pain, or feel unwell, seek emergency care now. If bleeding is lighter but pregnancy is possible, contact a clinician the same day. Record when it started, how it compares with your usual bleeding and whether pain is one-sided. Assessment may include pregnancy hormone blood tests and a Transvaginal Ultrasound.

Call emergency services for fainting, collapse, severe or sudden abdominal pain, shoulder-tip pain, breathlessness, confusion, cold clammy skin or heavy bleeding. These signs can occur with internal bleeding, which may not match the amount seen vaginally. Do not travel alone if you feel weak or dizzy.

Ask what the bleeding may represent, whether the pregnancy location is visible, when repeat testing is needed, and how to recognise deterioration while waiting.
