Left-sided pain during ovulation

Ovulation pain, also called mittelschmerz, can feel like a twinge, cramp, or dull ache low on one side. It may occur near the middle of the cycle and settle without treatment. The left side may be affected in one cycle and the right side in another, depending on which ovary is active.

Endometriosis and ovarian cysts can also present with one-sided pelvic pain. Symptoms such as pain outside the mid-cycle window, painful periods, pain during sex, bloating, fever, or unusual bleeding make a routine clinical review more useful.

Use gentle heat, rest, and pain relief that is suitable for you. Track the first day of each period, the day pain starts, its duration, side, severity, and any bleeding or discharge. Make an appointment if the pattern is new, repeatedly disrupts daily life, or does not settle as expected. If pregnancy is possible, ask about safe medicines.

Get urgent help for sudden severe left-sided pain, fainting, dizziness, fever, repeated vomiting, heavy bleeding, or a rigid or swollen abdomen. Seek prompt advice if pregnancy is possible or if pain is getting worse, because not every one-sided pelvic symptom is cycle-related.
