How long does ovulation pain last?

Ovulation pain, also called mittelschmerz, may occur around the middle of the menstrual cycle when an ovary releases an egg. It can feel like a one-sided ache, cramp, or sharper twinge in the lower abdomen. Some people notice it on the same side for several cycles, while others have no consistent side. Mild discomfort that settles on its own is often different from pain that continues, returns frequently outside the middle of the cycle, or interferes with daily activities. Endometriosis and ovarian cysts can also cause pelvic pain, so the timing alone cannot identify the cause.

Track when the pain begins, which side it affects, how long it lasts, and whether bleeding, discharge, nausea, fever, or urinary symptoms occur. Rest, gentle heat on the lower abdomen, and an over-the-counter pain reliever may help if these options are normally safe for you. Follow the product directions and check with a pharmacist or clinician if you are pregnant, have stomach or kidney problems, or take other medicines. Arrange a routine appointment when episodes are new, worsening, or difficult to manage.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked dizziness, heavy bleeding, fever, repeated vomiting, or pain with a positive pregnancy test. Prompt assessment is also appropriate when pain persists beyond the expected window or is accompanied by unusual discharge. These warning signs can have causes other than ovulation and should not be judged by how common ovulation pain is.
