Dry cough and wheezing at night: what should you know?

Wheezing is a musical or whistling sound made when air moves through narrowed airways. At night, bedroom dust, pet allergens, cooler air, reflux, or lying flat may expose or intensify an airway trigger. Asthma commonly presents with wheeze and cough that vary with time, activity or exposure.

An infection such as Acute Bronchitis can irritate the breathing tubes. Allergic Rhinitis may contribute through nasal inflammation and drainage. In babies and young children, Bronchiolitis can cause noisy breathing and feeding difficulty. COPD is another possible cause in some adults, particularly with a history of smoking or long-term airway symptoms.

Sit upright, move away from smoke or scented products, and use any prescribed reliever exactly as directed. Do not borrow another person’s inhaler. Note whether the sound is on breathing in or out, how quickly it began, and whether exercise, pets, dust or a recent infection preceded it.

A clinician may decide whether inhaled treatment, an allergy plan, or another evaluation is needed. Nebuliser Treatment is not automatically appropriate; its use depends on the person, the medicine and the severity of the breathing problem.

Call emergency services for severe breathlessness, inability to speak comfortably, blue or grey lips, confusion, collapse, or swelling of the mouth or throat. Wheezing that starts suddenly after food, medicine or an insect sting may be Anaphylaxis and needs immediate help. Seek prompt review for repeated nighttime episodes, chest tightness, fever with worsening breathing, or symptoms in a young child.

Ask whether the pattern could be Asthma, Acute Bronchitis, Allergic Rhinitis or COPD; whether you need breathing tests; and which action plan to follow if wheezing returns.
