Does inguinal hernia cause testicular pain?

An inguinal hernia occurs when tissue pushes through a weak area in the lower abdominal wall. Discomfort may spread from the groin toward the scrotum because the hernia lies near structures that pass into the testicle. Pain can become more noticeable with standing, coughing, lifting, or straining, and may ease when you lie down. A visible or touchable groin bulge can support this explanation, but testicular pain also has other possible causes, including infection, injury, or twisting of the testicle.

Arrange a clinical examination if the pain is new, persistent, or accompanied by a groin lump. Until you are assessed, avoid heavy lifting and activities that clearly worsen the discomfort. Do not forcefully push a painful or firm bulge back into place. A clinician may examine the groin while you stand and cough, and may check the scrotum to distinguish a hernia from another cause.

Seek urgent medical help for sudden or severe testicular pain, a testicle that sits higher or at an unusual angle, nausea or vomiting, marked scrotal swelling, fever, or a groin bulge that becomes firm, very tender, discoloured, or cannot be pushed back gently. These signs can indicate a time-sensitive testicular problem or a hernia complication.

Ask whether the pain pattern and examination fit an inguinal hernia, whether imaging is needed, which activities to pause, and how quickly treatment should be arranged if a hernia is confirmed.
