Many neck lumps have non-cancerous causes, including reactive lymph nodes, cysts, and thyroid conditions. The lump's presence alone cannot identify its cause.

Cancer of unknown primary describes cancer found away from its original site when the place where it began remains unidentified after evaluation. In the neck, abnormal cells may first be discovered in a lymph node. The location, firmness, mobility, growth pattern, and nearby symptoms help guide the assessment, but none of these features establishes a diagnosis by itself.

A clinician may examine the mouth, throat, thyroid, skin, and other lymph-node areas. Imaging and a sample of the lump can help identify the cell type and suggest where further investigation should focus.

Book an assessment for a new neck lump that persists, enlarges, feels hard or fixed, or returns after seeming to settle. Note when it appeared, whether its size changes, and whether you have pain, fever, dental symptoms, voice change, swallowing difficulty, ear pain, or unplanned weight loss.

Do not repeatedly squeeze or massage the lump. Bring a list of medicines, past cancers, recent infections, dental problems, and tobacco or alcohol exposure to the appointment because these details can shape the examination and testing plan.

Seek urgent medical help if swelling interferes with breathing or swallowing, grows rapidly, or occurs with drooling, noisy breathing, severe neck stiffness, or marked facial swelling. A persistent lump without these emergency features still warrants a timely clinical examination, especially when accompanied by a lasting voice change or unexplained weight loss.

Useful questions include which structures could be causing the lump, whether imaging or needle sampling is appropriate, what the sample can reveal, and which symptoms should trigger faster follow-up. If abnormal cells are found, ask how their features may guide the search for the original site.
