Difference between orthostatic hypotension and POTS

How the structures involved in Postural Hypotension differ from normal

Short answer

Orthostatic hypotension is a blood-pressure drop after standing, while POTS is an excessive heart-rate rise without that defining blood-pressure drop.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Orthostatic hypotension
Defined by a fall in blood pressure after standing
POTS
Defined by an excessive upright heart-rate response with symptoms

How the conditions differ

Orthostatic hypotension happens when blood pressure falls after moving from lying or sitting to standing. Reduced blood flow to the brain can cause light-headedness, blurred vision, weakness, or fainting. It may follow dehydration, illness, blood loss, or medicines that lower blood pressure. POTS, or postural orthostatic tachycardia syndrome, is a problem with the body's response to upright posture: the heart rate rises markedly and symptoms may include dizziness, palpitations, fatigue, shakiness, and difficulty concentrating. Blood pressure may remain stable, fall slightly, or vary, but a defining feature is not the same as orthostatic hypotension. Both conditions can coexist with other problems, so symptoms need measured assessment.

Useful next steps

Rise gradually, pause at the edge of the bed, and sit or lie down when symptoms begin. Avoid standing still for long periods and note meals, heat, illness, fluid intake, medicines, pulse, and symptoms. Do not increase salt, fluids, or compression treatment without individual advice, particularly if you have heart or kidney disease. A GP consultation can review medicines and arrange lying and standing measurements or other tests.

When urgent help is needed

Seek urgent care for fainting with chest pain, severe breathlessness, a sustained irregular heartbeat, serious injury, new neurological symptoms, or confusion. Get prompt medical advice if symptoms are rapidly worsening, you cannot keep fluids down, or you repeatedly nearly faint.

Questions for your doctor

Ask whether your symptoms suggest a blood-pressure problem, a heart-rate response problem, medicine effects, or another cause. Discuss what measurements are needed, whether a heart rhythm check is appropriate, and which activity, hydration, or compression measures are safe for you.

Questions people ask

Can POTS cause dizziness?

Yes. Dizziness, palpitations, fatigue, and reduced tolerance for standing are common reasons for assessment.

Can dehydration cause orthostatic hypotension?

Yes. Low fluid volume can make blood pressure harder to maintain when you stand, although other causes are possible.
